INCIDENT BRIEF The Motor Vessel OLMECA was transiting inbound Tampa Bay, Hillsborough Channel. When it came upon the turn at Cut "C" to Cut "D" the vessel steered wide and allided with the Motor Vessel Spar Three which was tied up to Pier 219. In an attempt to avoid the allision, the Master stopped engines and ordered full astern, causing the main engine to shutdown.

Direct drive diesels must shut down to go astern. Sounds like the engine failed to re-start in astern mode.
