A collision occurred at about 22.52 hours LT on the 15th of December 2007, between a Liberian Chemical Tanker vessel “FORMOSA TEN”, proceeding from Taiwan, China to MOKPO in Korea, and a local fishing vessel “ZHEFENGYU 11017”. The fishing boat sank and 19 crew members were lost. A further crew member was rescued.

From interviews with the crew involved in the accident, and records of the relevant evidence, it was concluded that the duty officer of the FORMOSA 10 had been neglectful of look out and had not taken effective measures to avoid the collision. The fishing vessel had also failed to take action to avoid the collision. Full details are given in the investigation report.

The GISIS entry above is everything GISIS has on this killer casualty. Presumably, it is from the flag state, Liberia. It tells us just about nothing. If there is an investigation report, GISIS does not have it, nor can we find it.

Obviously, collision reports should give the tracks, the conditions, communications, damage data, etc, etc, whatever is known about the casualty. But we have nothing other than 'human error'.
