M/T Quetzalcoatl exploded at the Pemex-terminal in Veracruz Oct 17 -- 8 dead and 9 injured. About 400 persons in the area of Coatzacoalcos were evacuated. The explosion occurred while or after unloading and caused by welding works on a nearby crane.

On October 17,2006, an explosion occured at PEMEX's Quetzacoatl tanker. The incident occurred while the contracting company, Lopez Garcia SA, was repairing the loading crane of the ship at the Pajaritos Terminal in Coatzacoalcos, Veracruz. The tanker was not in operation and was not loaded with any fuel. The explosion was caused by a spark that had contact with gas residuals in the ventilation system of the tanker. Seven people were killed in this incident.

Photo clearly indicates crane was on-board. Fire was midships in the vicinity of the crane. Looks like lousy inerting, ventilation, during hotwork.
