Vessel failed to report multiple cracks
in No 6 cargo tank and cargo migration.
Master failed to inform class of repairs to cargo tank.
Company failed to take timely corrective action and make
permanent repairs to cargo tanks.
Ship is 1988 blt double hull 94,000 tonner.
Other sources claim ship had made temporary repairs with epoxy.
Might be fatigue cracks or corrosion from the ballast side high.
But this is not confirmed, nor how the USCG inspection got into the tank.
