Randgrid, a shuttle tanker was properly secured to the Tetney monobuoy and discharging. Since she was a shuttle tanker she had a remote chainstopper release on the bridge. This is extremely unusual. Crew, probably tired, wanted to secure the forward hydraulic pump -- also remotely -- but instead released the chainstopper. The two functions used the same function key, but on different screens on the monitor. Ship drifted way from the buoy and the hose broke. Approximately 12 tons spilled. The computer interface was poorly designed. Much better to have mimic boards.
