All 30 members of the crew of an Italian motor bulk carrier Marine di Equa lost their lives when they abandoned ship after reporting serious leakage in a hold during storm force 10 winds and very heavy seas of the NW coast of Spain in lat 45,35N, long 11.00W on December 29, 1981. A search was conducted for survivors from the vessel, which had been on voyage from Antwerp to Houston, loaded with steel plates and coils. But even though liferafts were spotted, no crewmen could be located.

The use of the adjective open is ambiguous. Could mean hatch cover gone, could mean the hull is holed. For now calling this a hatch cover failure.
